Diagnosis and management of retinoblastoma in Kenya, and many other developing countries, fails to reach the optimal outcome due to lack of guidelines and clearly defined treatment protocols.

Due to its rarity, little published literature is available regarding appropriate management of retinoblastoma in resource-limited settings.  Limited professional development opportunities have also added to lack of agreement on the best way to manage children.

For example, diagnosis of retinoblastoma must be determined at the clinical level.  Biopsy is contraindicated in retinoblastoma as this surgery can lead to tumour spread along the biopsy needle track.  Pathological diagnosis is safe only after surgical removal of the entire eye.  The timely pathological examination of the eye with retinoblastoma is very important in the planning of further treatment to save the child’s life. 

We will work with the Kenyan National Rb Strategy Group to introduce guidelines appropriate for the local setting.  The guidelines will provide agreed, evidence-based recommendations for management of patients and their families, from diagnosis and referral, through treatment and into follow-up.

Using the Canadian Retinoblastoma Best Practice Guidelines as a model, we will

assist the KRbS Group with adaptation and development of guidelines for local use; 

facilitate national meetings to enable a collaborative approach that ensures consensus and comprehensive ownership; and

provide knowledge and expertise in guideline writing to facilitate appropriate editing and formulation of the document.

Best Practice Guidelines will establish common standards of care and facilitate appropriate management of retinoblastoma.  In the process, financial, physical and social burdens currently experienced by families will be reduced, and access to appropriate care will be increased.
